PREDMETH: methotrexate and prednisone in the treatment of pulmonary sarcoidosis

PREDMETH: in people with pulmonary sarcoidosis, methotrexate achieved a similar effect on lung function to prednisone after 24 weeks. The trial compared two initial treatment options, but this does not mean that either is suitable for everyone.

138 adults with pulmonary sarcoidosis who had not previously been treated for the disease were enrolled in the randomized trial. Participants received methotrexate or prednisone. After 24 weeks, methotrexate was not inferior to prednisone according to the prespecified measure of lung function (forced vital capacity, FVC). This supports considering methotrexate as one of the initial options for some patients, especially when steroid side effects are taken into account.

The trial was open-label and short-term; it included a selected group with pulmonary disease. The result does not determine the best medicine for an individual and cannot be directly applied to every form of sarcoidosis. Treatment is chosen with a doctor based on the organs involved, disease severity, other conditions, and possible side effects. Do not change your treatment on your own.
